Clinical Requirements...
The School of Nursing has a contract with each of the clinical agencies used for the various clinical sites. These contracts indicate what health documentation is required of all students at the various agencies. Following is health documentation information that you need to provide for the school’s official files.
Information about equipment that students will need when in the clinical setting is also included.
Health Documentation:
- Health Insurance
- Proof of Health Insurance is required and must be submitted each academic semester and will be kept in the student’s file. No student will be permitted in clinical courses without proof of health insurance.
- CPR
- Basic Life Support (BLS)
- CPR Certification
- The Healthcare Provider Level is required
- Health Status Certification
- The following are required and they must be completed prior to the clinical experience:
- Provide documentation of physical examination within six months prior to entering into the program.
- Tuberculosis: Must do either (a), (b), or (c):
- (a) Two-step TB skin test (PPD) for students with no history of receiving a PPD or have not been tested in the last 12 months;
- (b) Proof of a negative Tuberculin-PPD in the last 12 months and an original negative two-step;
- (c) Document of a negative chest x-ray within the past 5 years for students with a history of past positive PPD
- All students with a history of a past positive PPD must complete the tuberculosis signs and symptoms questionnaire available in the nursing program office
- Rubella: documented receipt of one vaccination after first birthday, serology or born before 1957.
- Rubeola: documented receipt of two vaccinations on or after first birthday, serology or born before 1957.
- Chicken pox: history or documented receipt of vaccination
- Hepatitis B: vaccines or titer or statement of refusal (declination)
- Tetanus and diphtheria: inoculation within past ten (10) years
- Negative result to an 8-panel drug screen.
- Physical Exam. A physical exam conducted by a primary care provider is required upon entry to the nursing program. This Physical Exam Form is submitted in its original (not a copy) form to the student’s advisor. Students need to use the School of Nursing’s form.
- Health History Form. This is a health history questionnaire that the student completes. It is available on the Forms page.
Students will not be admitted to clinical practice without documentation on file in the nursing office.
Clinical Equipment
Students are required to have the following available for clinical use:- Bandage scissors
- A watch that indicates seconds
- Pen light
- Stethoscope which should have both bell and diaphragm.
- White laboratory coat (no knit cuffs), the designated NSC uniform, and name badge. (Purchased from Dove Professional Apparel)
- White closed heel shoes.
- Nevada State College patch. Patch will be placed four inches below the shoulder seam on left arm of the uniform and lab coat. (Purchased from Dove Professional Apparel)
- Name Badge. (Purchased from Dove Professional Apparel)
