Completed forms may be turned in to the Office of the Registrar in person, via fax or mail. Please know that forms will not be accepted through e-mail or by phone. Incomplete forms will not be processed.
Provide the Nevada State College issued Student H-number, not a Social Security Number when asked for identification. The Student H-number is available through
Student Account. All forms must be completed by printing with a blue or black inked pen. Illegible forms will not be processed.
Note: It is the student’s responsibility to ensure that all forms are completed fully and correctly and submitted in a timely fashion within all college deadlines.

Description: A signature from the student’s current adviser and approval from the new major department are needed to complete this form. After all signatures have been obtained, turn the form into the Office of the Registrar for the official change. Processing time: two weeks from receipt of completed form.

Description: Completely fill out the request and submit it to the Office of the Registrar. Only the number of credits and student status will be verified by the enrollment request. If verification that the bill has been paid is needed, it may be requested from the College of Southern Nevada’s Bursar’s Office by calling (702) 651-5500. Also, if grades or grade point average verification is required, please request a transcript. Processing time: two business days from receipt of completed form.

Description: All students must apply for graduation in order to receive an official degree evaluation from the Office of the Registrar. Applications must be received by the following deadlines: if graduating in spring or summer, then the deadline is Oct. 1; if graduating in the fall, then the deadline is March 1. Please pay attention to each semester’s specific deadline. Completely fill out this form, including intention to attend commencement and which ceremony. Please know that your degree is a bachelor of arts (BA), bachelor of science (BS), bachelor of public administration (BPA) or bachelor of applied science (BAS). Major refers to the specific course of study, such as nursing, elementary education, English, etc. Once received, the Office of the Registrar will do an official degree check. It will be sent to the major department, who will approve or change the requirements remaining and send it back to us. We will then send you a letter stating all of the requirements that must be met for degree completion. Processing time: three to four weeks from receipt of completed form.

Description: This form is used to update the semester in which a student intends to complete degree requirements, update the semester in which a student plans to attend commencement, cancel a previously received graduation application or update a student’s expected graduation date. Processing time: One week from receipt of completed form.

Description: Completely fill out the request and submit it to the Office of the Registrar. Please include legible copies of all the required documentation, which may include a driver’s license, Social Security card, marriage license and others. A full list of options and combinations available are listed on the form. Please be advised that any name changes submitted near the end of the semester will not be processed until all grades have been submitted. Processing time: One week from receipt of completed form.

Description: This form is used to add or change a minor program to the student's degree. The available minors and their codes are listed on the form. The student will write the desired minor code into the indicated box, whether they are adding or dropping, and obtain the signature of their advisor in their major program.
